INTERNET APPLICATION AND SUBMISSION OF REQUIRED DOCUMENTS DECLARATION

1. I hereby apply for a loan from Promise (HK) Co., Ltd. (hereinafter called "Promise"). For the purpose of showing my ability to repay the loan and inducing Promise to advance the same, I hereby acknowledge that Promise in advancing the said loan will rely on all information and required documents (including but not limited: HK Identity card, bank account proof) provided and declared by me which shall be true and accurate; and also acknowledge that such information (including required documents) shall be the basis on which the said loan is made.
2. Before providing the said information, I confirm that the contents of the Personal Information Collection Statement ("PICS") as provided by Promise have been received and understood.
3. I understand that I may be asked to provide personal data of certain third parties to Promise in relation to this loan application and that the Personal Data (Privacy) Ordinance (Cap. 486) ("PDPO") imposes certain requirements upon the collection of personal data. For these purposes, I confirm that I have complied with the PDPO on collection of personal data of the third parties, including, but not limited to, informing the third parties (a) what data will be provided to Promise, (b) they are not obliged to supply their data; (c) if they agree, I will disclose their data to Promise; (d) their data will be used for the purposes of the Loan Agreement and the loan application process; and (e) of their rights to request access to and to correct data Promise holds about them, and the details of Promise's Data Protection Officer who handles such requests and which are stated in the Promise's PICS. I further confirm that a copy of the PICS was provided to each of the third parties whose personal data were disclosed in the loan application process and consent was obtained from each of them for this purpose.
4. Furthermore, I agree that Promise may verify the truthfulness of the said information from any sources. I also understand that, whether the said application is to be approved or not, this application form and required documents as provided remain the property of Promise.
5. I hereby agree that Promise is entitled to request me to provide further information and documents in support of my application. All documents including this application form shall not be returnable.
6. I hereby agree that approval of this loan application including the loan amount granted, annual interest rate and number of instalments shall be at the sole discretion of Promise without assigning any reason.
7. I declare that I have not been delinquent in repaying any credit facilities with any financial institution. I am not bankrupt or a discharged bankrupt, have no intention to declare bankruptcy, and am not aware of any bankruptcy proceedings made against me.
8. I agree that transaction via internet is a new and rapidly advancing technology. Accordingly, there may have limitations on internet transactions where my expectation might not be met. While I understand that Promise will use its best endeavours to keep my personal data confidential, Promise shall not be liable for any claim, loss or damage of any leakage of my personal data which is not caused by unauthorized disclosure of Promise.
9. I agree that Promise shall retain all records relating to transactions from Online System and 24-hour IVR Telephone System. Such records are deemed to be legal documents which shall be binding on me.
10. I confirm that I have read the PICS and Privacy Policy Statement over the internet and agreed the same. Moreover, I agree all the declarations as stated in the loan application form and agree to comply with all terms and conditions of the same.
11. I further understand and accept the content of this Declaration, and shall submit the form and required documents as provided herewith.
